Positional T wave Change on Electrocardiogram During Operation - A case report -

Abstract
Accurate detection of the earliest signs of ischemia on the surface electrocardiogram (ECG) is essential for timely diagnosis and management of potentially life-threatening ischemia event. Yet, the accuracy of ischemia analysis in ECG monitors remains suboptimal because of a number of confounding factors, including changes in body position and
other artifacts. A 55-year-old female with right pyohip was scheduled for curettage and debridement in left lateral position under epidural anesthesia. During the operation, the patient experienced tingling sensation of right upper extremity, chest tightness, deep T wave inversion, and stuporous mental change after second epidural injection. We managed
patient correspond to myocardial ischemia. However, T wave was normalized after position change into supine.
We should anesthetize considering the positional ECG change. During operation, anesthesiologists need to closely monitor the ECG change after the cause of myocardial ischemia is offered, not simply abnormal wave, and actively cope with it.
